Summary:

This prospective randomized pilot study compared the long-term regenerative efficacy of autologous platelet-rich plasma (PRP) injections versus phonophoresis with kinesiotaping (KT) in patients with chronic plantar fasciitis unresponsive to conservative treatments. Out of 64 enrolled patients, 54 completed the 24-week follow-up. While KT provided better early pain relief at 2 weeks, PRP showed significantly greater improvements in pain (Numerical Rating Scale), foot function (Foot Function Index), and plantar fascia thickness at 12 and 24 weeks. The study used two ultrasound-guided PRP injections spaced two weeks apart, with a standardized preparation method ensuring high platelet concentration and minimal contamination.

The results support that PRP not only reduces symptoms but also promotes tissue regeneration in plantar fasciitis. Unlike KT, whose benefits were short-lived, PRP demonstrated sustained pain relief and structural improvement over six months, with no reported adverse effects. The study suggests that therapeutic-quality PRP injections can be a viable, cost-effective, and safer long-term option for managing chronic plantar fasciitis, especially in patients who have not responded to traditional therapies.
