Platelet-Rich Plasma Treatment With Physical Therapy in Chronic Partial Supraspinatus Tears

Summary:

This randomized controlled study compared platelet-rich plasma (PRP) injections with standard physical therapy (PT) in 70 patients with chronic partial supraspinatus tears confirmed by MRI. Patients received either three weekly intra-articular PRP injections or 15 PT sessions over three weeks. Outcomes were tracked before treatment, immediately after treatment, and 12 months later, including pain (VAS), function (DASH questionnaire), shoulder range of motion (ROM), and clinical shoulder tests.

Both PRP and PT groups experienced significant long-term improvements in pain, ROM, and shoulder function. PT led to greater gains in shoulder motion and faster short-term pain relief, likely due to the structured exercise and nerve stimulation. However, PRP patients achieved significantly better improvements in functional disability (DASH scores), showing lasting benefits in daily activities. PRP was well tolerated, with no major side effects, making it a safe, autologous treatment option with promising potential for chronic partial supraspinatus tears.
